Waiver Requests

USPA members can request exceptions to USPA policies for reasons of hardship or in the interest of development of new technology. Follow the procedures outlined in Skydiver's Information Manual Section 2-2.

Waiver request by e-mail

1. Fill out this waiver request form in Microsoft Word.
2. E-mail one copy to your USPA Regional Director (Click here for a list of USPA Regional Directors.).
3. E-mail one copy to USPA Headquarters.
