How to Apply for Achievement Awards
Members can apply for Jump Wings, Freefall Badges, Performance Awards and Tenure Certificates themselves.

Current USPA members can apply for Jump Wings after accumulating 1,000 jumps, 2,000 jumps, 3,000 jumps and so on.
To apply for Jump Wings Awards:
Freefall Badge Awards
Current USPA members can apply for Freefall Badges after accumulating 12 hours of freefall time, 24 hours, 36 hours, and so on.

Performance Awards
Canopy Formation Awards
Awarded to current members who participate in the following canopy formations:
- Four-Stack (Day or Night) Award: Take part in a canopy formation of four or more people
- Canopy Crest Recipient (Day or Night) Award: Take part in a canopy formation of eight or more people
- Canopy Crest Soloist (Day or Night) Award: Join as the 8th person or later into a canopy formation of eight or more people
To be eligible for an award:
- All participants must be active members of USPA, or if not a U.S. citizen, an FAI-partnered aero club at the time of the jump
- Formation must be planned in advance
- Formation must be described in the SCM or recognizable (e.g. a diamond)
- Completed formation must be held for a continuous 10 or more seconds
To apply for a Canopy Formation Performance Award:
- Apply Online
- Verify all participants are active members of USPA or an FAI-partnered aero club at the time of the jump
- Visit the My Awards page
- Navigate to the Performance Awards tab
- Click the "Apply for Performance Award" button
- Complete and submit the form
- Application Form
- Verify all participants are active members of USPA or an FAI-partnered aero club at the time of the jump
- Download and complete the application form
- Send completed form to Member Services
Tenure Certificates
Awarded to annual and lifetime members beginning with 10 years of total active membership and every 5 years of total active membership thereafter. Lapses in membership are not included in this total. Membership may need to be manually reviewed, especially if membership began before USPA’s digital systems were established. When verified, a certificate can be mailed to the recipient.
To apply for a Tenure Certificate:

- Send the completed form to Member Services
